The above named player has my permission to participate in the Jaguar Basketball League.  While I expect volunteers and coaches to exercise every reasonable precaution to avoid accidents and injury, I hereby release the Cherry Hill Board of Education, School Age Child Care Program, Student Enrichment program, and the Cooper PTA including but not limited to their employees, volunteers, directors, supervisors, referees, and coaches of any liability whatsoever for accidents or conflicts that may occur during participation.  By signing this form you are certifying that the player (child) is physically able to participate in this rigorous activity and that the child has no physical restrictions that would prevent the child from participating.

I am aware that Jaguar Basketball League is only providing supervision for those players directly involved in their scheduled game(s).  It is the responsibility of parents/guardians to supervise their young people before and after the scheduled game(s) and to arrange for their transportation.  This is not a school or PTA sponsored team.
